Your child's participation in the Inter-City Catholic League (I.C.C.L.) requires the completion of the following forms:
For the 2021 spring sports season, the Emergency Medical Form can be completed and given directly to your child's coach.
The I.C.C.L. Athletic Contract requires both parent and player signatures and should be sent to the school office.
